Welcome for Transitional Care Funding & Liaison with Nursing Homes

Tadhg Daly, NHI CEO states: “We welcome the provision of an additional €1.5 million for transitional care. The majority of people ready for discharge in our acute hospitals require nursing home care. Private and voluntary nursing homes will facilitate over 10,000 discharges from acute hospitals back into the community this year. Without timely access to nursing home care, the pressures presenting for our acute hospitals will multiply. The commitment to assign a staff member to support admission avoidance from nursing homes, where possible, is also welcome. However, they should also fulfil a lead role in facilitating the timely transfer of patients from hospitals to nursing home care. The Department’s recently published report on delayed discharges highlighted significant impediments that are preventing patients from moving to nursing home care in a timely manner. To support efficient operation of our acute hospitals, it is imperative underlying issues in discharging patients to nursing home care, where required, are addressed.”
